Pre Admission

Our aim is to make sure you have the best possible preparation for your surgery and stay. This process begins with having all your paperwork completed and returned so we can plan your admission, stay and discharge. It also means that we can inform you of how much your hospital stay will cost.

Once the decision has been made that you require admission to Ramsay Surgical Centre Glenferrie, your doctor will provide you with an admission pack. You can either fill out this paperwork and send it back to us as outlined below, or you may use our On Line Booking Service.

Have the following information ready when you fill in your preadmission information:

  1. Personal and Next of Kin details
  2. Medicare Card
  3. Funding details (eg Private Health Insuance details, DVA, Workcover or Self Funding)
  4. Benefit details (eg Pension or Health Care Card)
  5. Item numbers, if these were quoted by your Doctor’s rooms
  6. Information your Doctor supplied to you regarding implantable disposable devices (eg prosthetic and disposable), if applicable

If you choose to fill out the paperwork you are required to:

  • Complete all questions on the Pre-Admission Form and Patient History Form
  • Read and sign the Ramsay Health Care Privacy Policy and sign that section on the Admission Form
  • Sign the Consent Form.  Ensure that you and the doctor sign and date the consent form.

Remove the completed forms from the Admission booklet and forward immediately to the hospital in one of the following ways:

Hospital Bookings are only confirmed on receipt of Pre-Admission paperwork and should be returned at least 7 days prior to admission.

Pre Admission Service

The Pre-Admission Service operates from 0900 to 1400 Monday to Thursday. The aim of this service is to ensure that you are fully prepared for your surgery and stay, and for your discharge.

The completed Admission Forms and Consent Forms are forwarded to the Pre-Admission Coordinator for processing.

Once the Pre-Admission Coordinator has reviewed your information she will telephone you to go through the information. They will discuss your expected length of stay and any discharge plans that may need to be made.

You will also be given a time for your admission on the day of surgery.

The Preadmission Coordinator will inform you of any out of pocket costs related to your stay. Out of pocket costs are those costs that are not covered by your private health insurance.
